- Charges for therapy:
Please see charges within our site.
- Payment methods for patients not insured:
We request that payment is made following each treatment session. This may be by debit/credit card, (not AMEX) or cash.
- Payments through Insurance companies:
Farnham Physiotherapy & Sports Clinic Ltd are registered with all major medical insurance companies. We will invoice the insurance company direct on your behalf provided you give us the appropriate policy/authorisation numbers at your FIRST APPOINTMENT.
- Late payments may result in an administrative charge being added:
If overdue invoices or letter are not settled, we may use a debt recovery service.
- Non-attendance and cancellation:
If you do not attend or do not cancel your appointment at least 24 hours prior to the appointment time, the full charge for the appointment will be made unless exceptional circumstances apply.
- Access to medical records:
Farnham Physiotherapy & Sports Clinic Ltd will supply photocopies of records to a third party subject to receipt of the request in writing with your full signature. There will be an administration charge of £50.00 for this service.
- Discharge report:
As a matter of good practise your physiotherapist we may send a discharge report/letter to your general practitioner or consultant. Please inform your physiotherapist if you do not wish them to do this. For any request of report from patient to insurance company or any other relevant professional, an administration fees of £25 will be charged.
- If you have a complaint, please contact the office by telephone (01252 726479) or in writing to: The Practice Manager, Farnham Physiotherapy & Sports Clinic Ltd, 20 Firgrove Hill, Farnham, Surrey GU9 8LQ.
- Children (under the age of 16) must be accompanied by an appropriate adult (parent or guardian). Where no appropriate adult has accompanied them to the clinic, a member of the clinic staff will be present in the room during the treatment or, is not available, an alternative appointment will need to be arranged.
- A culture of openness between patient and therapist in actively always encouraged. We endeavour to provide patients with any information required about what to expect when they attend our practice, and we ask for their permission to assess and treat prior to the first appointment on the written consent form.Procedures such as manipulation, acupuncture and specific examination techniques require extra care to ensure that patients understand what they are consenting to. Informed consent to all treatment and assessment is vital and includes an explanation of pros and cons in language that patients can clearly understand.Any treatments or examinations can be discontinued at any time if patients request this even if written consent has been given.
Patients are encouraged to maintain independence and self-care as far as is practicable, for example undressing themselves. Assistance is only given if it has been clarified that this is required.
